Guidelines for sending the street monitor form:

1. Fill in the form. You may need to delete text if it still showing from a previous submission. Please send in the form every week, even if you have nothing to report (in this case select the 'nothing to report' box on the form).
2. Once the form is filled in, scroll down and press the 'Submit by Email' button. You can select 'Print' if you want a copy for your own records.

The form will then default to your email system. Select 'Desktop Email Application' or 'Internet Email' or 'Other' as appropriate, then click OK.
4. Select 'Send Data File'.
5. The form will then be sent as an email attachment to Alexa Tamsitt, Environment Administration Worker.
If you have any problems with the form please call Janet Deverell, Communications Officer, on 377 3639. We are currently working on making the form compatible with Apple Mac computers.
Calling Customer Services:
Please be aware that when you call Customer Services (922 3838) to report problems they have a tick box for street monitors (or reps as they are called elsewhere) so please ask that they tick this box when you make your call. Please ask for a reference number beginning '101' as it is only this number that can be used to chase up jobs requested.
